This notice explains how Dentistry on Britannia handles personal information collected through this website. It does not cover the clinical records we keep as your dental care provider, which are governed by Ontario’s Personal Health Information Protection Act (PHIPA) and by our in-practice privacy policy. Ask at reception for a copy.
What we collect
When you submit the appointment request form we collect the information you enter: your full name, phone number, email address, appointment type, optional reason for your visit, and your consent for us to contact you about the request.
Our server also records the time of submission and the IP address the request came from. We use that only to prevent automated abuse of the form.
Why we collect it
Solely to respond to your request and arrange an appointment. We do not sell your information, and we do not use it for marketing unless you separately ask us to.
How long we keep it
Form submissions are retained only as long as needed to arrange and confirm your appointment, after which the relevant details are transferred into your patient record and the email copy is deleted.
Who can see it
Form submissions are emailed to our practice and are accessible only to the staff who schedule appointments. The email is transmitted over an encrypted SMTP connection.
Cookies and analytics
This website sets no advertising or tracking cookies of its own. The map on our contact page is embedded from Google Maps, which may set cookies under Google’s own privacy policy.
Your rights
